Dear Patients

As you will be aware, the COVID-19 rates of infection have increased to significant levels in our area. At the same time we need to maximise our work of vaccinating patients with the COVID-19 vaccine. We have been advised to free up time with our team to help look after patients with emergencies and protect patients with the vaccine. If your problem is an emergency then please call 999 If you need to contact the practice with other symptoms or queries please use the eConsult 'get started' link above. Your online form will be reviewed by a clinician and you will be contacted back with a text. You will either be contacted by the surgery to ask you more questions or you will receive a text on what to do next.

MEDICAL CERTIFICATES FOR SELF ISOLATION

Medical Certificates for first 7 days are not required. No Medical certificates will be provided by the practice for self-isolation past 7 days.  COVID-19 related sick notes are available here: https://111.nhs.uk/isolation-note

COVID-19 VACCINATION STATUS LETTER

GP practices cannot provide you with a COVID-19 vaccination status letter. You can demonstrate your vaccination status by downloading the NHS App. It may take more than a week for your identity to be checked and verified. For more information visit www.nhs.uk/nhsapp.

If you do not have internet access, have had both doses of your COVID-19 vaccination, and are due to travel outside the UK in the near future, you can request a paper letter from the NHS by calling 119. Only call 119 if you had your second dose more than 5 working days ago. It may take 7 working days for the letter to arrive.

You can now request your repeat prescriptions and book your appointment with a doctor online. In order to do this, you need to register for Patient Access by coming into the surgery and requesting your unique registration password that will be personal to you. Alternatively please download the NHS App onto your mobile phone.

Health and Social Care Information Centre (HSCIC)

The HSCIC will shortly begin extracting data via GP Extraction Services (GPES). For further information please see page dedicated to this. Leaflets are also available in the surgery.
